Job Title: Care Coordinator

Department: Clinical Resource Management

Reports To: System Manager of Care Management

Job Summary:

Cayuga Health System is seeking an energetic and motivated individual in search of part-time, benefited, employment in this newly created position. The Care Coordinator will play an integral role in a number of activities in the promotion of safe patient care transition. The care coordinator collaborates with providers & practices (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery.

Applicant should have strong analytical, problem solving, critical thinking, and organizational skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with system stakeholders and partners as assigned. The Care Coordinator also serves the needs of the department to optimize efficiencies for team members and supervisors. Must be able to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity.

Job Responsibilities include:

  • Assess the patient's health history and facilitate insurance navigations and primary care engagement as indicated.
  • Demonstrates understanding of palliative/hospice/home care referral requirements and appropriately facilitates referrals as indicated.
  • Demonstrates knowledge of and collaborates regularly with community-based organizations and partners.
  • Appropriately documents client interactions and intended plan of care.
  • Routinely communicates with patient family/caregivers/decision makers as indicated.
  • Monitors and appropriately escalates issues affecting patient safety and/or length of stay. Actively intervenes to facilitate patient progression and safe care transitions.
  • Works in collaboration with ancillary personnel (i.e. physical therapy, pharmacy, diabetes education) to support identified patient care needs.
  • Attends multidisciplinary rounds, working with the team to develop safe/sustainable transitions of care.
  • Facilitates patient access to appropriate providers and service organizations.
  • Supports administrative functions of the Clinical Resource Management Team.
  • Appropriately recognizes and responds to opportunities for improvement.
  • Practices within scope - activities may be carried out within the role provided you have the knowledge, skills, and experience to do so lawfully, safely and effectively.
  • Participates in organizational and community committees as assigned.
  • Maintains high visibility and is readily approachable.
  • Seeks learning opportunities to enhance job performance.
  • Communicates effectively with care teams, patients, families, community partners, and guests.
  • Remains calm in stressful scenarios and able to resolve conflict independently, appropriately escalating concerns as indicated.

  • Appropriately shares knowledge with coworkers.
  • Demonstrate strong written and verbal communication skills. Enforces appropriate written and verbal communication among team members.
  • Exemplifies satisfactory attendance and punctuality record as set forth by CHS policies.
  • Exemplifies a professional image in appearance, manner and presentation.
  • Maintains patient confidentiality in the provision of high quality care.
  • Is flexible in assuming other responsibilities not noted above.

Requirements:

Education - Associate's Degree preferred, will consider High School Diploma with 5+ years of relevant experience in healthcare.

Experience - Experience in the acute care setting, or related healthcare field, is preferred. Demonstrated knowledge of Microsoft products and zoom.

Licensure - Not required.

Physical Requirements - Ability to stand, sit or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base.
