Outpatient Staff Pharmacist - Per Diem

Staff Pharmacists/outpatient fill orders for medications, monitor patient medication therapies, provide drug information, and supervise and direct support personnel. In the absence of the outpatient pharmacy manager, a staff pharmacist/outpatient may be required to assume the essential responsibilities and perform the duties of the outpatient pharmacy manager.

Roles and Responsibilities:

• Interprets medication orders and transcribes to computerized patient profiles accurately, maintaining complete patient medication profiles.
• Prepares and dispenses medications, including sterile IV preparations, using proper techniques.
• Issues controlled substances to patient care areas and maintains records as required by law.
• Monitors medication therapy regimens for interactions, allergies, appropriateness, and therapeutic duplication.
• Provides clinical consultation, recommends evidence-based medication regimens, and suggests cost-effective therapeutic alternatives.
• Attends ABC alert codes, assists with pharmacokinetic and anticoagulation consult services, and reports adverse drug events and medication errors accurately and timely

Required Skills and Experience:

• Education - Graduate of an ACPE-accredited school of pharmacy with either a BS pharmacy or PharmD required.
• Licensure - Currently licensed in the State of New York.
• Demonstrates flexible in assuming other appropriate responsibilities

Preferred Skills and Experience:

• Health-system pharmacy experience preferred, but not required.
