SUMMARY:

The Patient Safety Investigator will play a pivotal role in advancing the safety of the Behavioral Health Service lines across Centralus Health. This position requires a highly skilled, autonomous professional and detail oriented person who would be responsible for conducting thorough investigations into safety events, including incidents, near misses, and adverse events.


The Investigator will interview involved parties, gather and analyze information, and identify root causes. The person in this role will work with Risk and Patient Safety professionals and Operational leaders to develop, track and close actionable recommendations for preventing future occurrences. This role requires an excellent communicator who can foster a culture of safety, trust and psychological safety. The Investigator will be responsible for investigations that fall within the jurisdiction/oversight of the New York State Office of Mental Health (OMH) and the New York State Office of Addiction Services and Supports (OASAS) with frequent contact with regulatory professionals and offices across New York State.

The Patient Safety Investigator will support Cayuga Addiction Recovery Services, Behavioral Health Units for hospitals and other programs under OMH and OASAS across Centralus Health. This role involves working closely with healthcare providers to inform the Incident Review process and promote a culture of safety and continuous improvement, specifically within the regulatory functions of OMH and OASAS.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Conduct Investigations: Lead comprehensive investigations into patient safety incidents that fall within the OMH and OASAS regulatory functions, utilizing evidence-based methodologies and tools.
  • Case Analysis: Review and analyze cases to identify trends, patterns, and areas for improvement.
  • Data Visualization: Responsible for maintaining a comprehensive dashboard that would aid in assessing the volume, trends, and improvements rooting from incidents that have been investigated.
  • Documentation: Maintain accurate and comprehensive records of investigation activities and outcomes.
  • Collaboration: Work collaboratively with healthcare providers, clinical staff, and administrative personnel to implement recommendations from investigations.
  • Incident Review Process: Inform and support the Incident Review process by providing detailed analysis and insights from investigations.
  • Education and Training: Provide education and training to clinical staff on patient safety processes and risk management strategies.
  • Teaching and Coaching:. Provide ongoing coaching and support to healthcare providers to foster continuous improvement and adherence to best practices.
  • Compliance: Ensure compliance with regulatory requirements and organizational policies related to patient safety and clinical risk management.
  • Reporting: Prepare and present reports on investigation findings and recommendations to the Incident Review Committee and management.
  • Other duties as assigned.



QUALIFICATIONS AND REQUIREMENTS:

Education: Bachelor's degree in Nursing, Healthcare Administration, Care Management/Social Work, or a related field. Advanced degree preferred.

Experience: Minimum of 5 years of experience in clinical risk management, peer review, or a related area within a healthcare setting.


Skills: Strong analytical skills, excellent communication and interpersonal abilities, proficiency in data analysis and reporting, and familiarity with healthcare regulations and standards.

Certifications: Office of Mental Health and Justice Center Training Certification


Skills, Knowledge, & Abilities:


Travel: Required within the various CHS locations.


Work location: In-person/ Hybrid


Physical Nature of the Job: Sedentary work: Exerting up to 10 pounds of force occasionally in carrying, lifting, pushing, pulling objects. Sitting most of the time, with walking and standing required only occasionally.


Salary Range: $65,000 - $85,000
