Job Title: Triage RN

Department: Schuyler Hospital Clinics

Reports To: Manager Primary Care and Specialty Services, Schuyler Hospital

Job Summary: This position supports triage functions and care coordination for patients of the Schuyler Rural Health Clinics in alignment with CHP network strategic goals and partnerships. The RN collaborates with providers (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong analytical, problem solving, critical thinking, organization and decision making skills. The RN must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with Schuyler Hospital, CHP network stakeholders and partners as assigned. Must be able to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity.

Job Responsibilities include:

• Promote the mission, vision, and values of Schuyler Hospital & Cayuga Health Partners.
• Performs standard nursing assessments on all patients based on clinical protocols including but not limited to health history, vital signs and applicable screenings.
• Responsible for supporting triage for primary care, and specialty services
• Responsible for the collection of any quality improvement and clinical outcomes data related clinical processes performed at Schuyler Hospital as defined by the physicians, clinic leadership and organizational goals.
• Appropriately documents all encounters with patients.
• Appropriate communication with network partners, providers, patients, caregivers, and team.
• Practices always within scope - activities may be carried out within the role provided you have the knowledge, skills, and experience to do so lawfully, safely and effectively.
• Other duties as assigned

Requirements:

Education - Associates of applied science of nursing from accredited institution. Registered Nurse (RN)

Experience - Minimum of 1-year med/surg nursing experience.

Licensure - RN

NYS registration to preform duties of a registered nurse.

Physical Requirements -Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Repetitive use of hands and fingers (e.g., use of computer keyboard); may require lifting and carrying light loads (up to 40 lbs.), including boxes, equipment, unit-dose cassettes, and IV solutions and stooping and kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers); sitting, walking, or standing for long periods of time (4-8 hours) is often necessary; must be able physically to operate the equipment used for the job.
